I would like to participate in Operation Goody Bag’s Service Learning project.  My goal is to work towards the Bronze Award which is a total of 50 – 74 hours.  I will check Operation Goody Bag website for meeting dates and announcements. I will attend the cafeteria after school meetings ONLY when I can stay for the full 45 minutes.  I will bring my own crayons, markers and pens.  Mrs. Cosco will supply brown lunch bags and personal letter forms for my use.  I realize that only quality work will be credited towards my time allotment.  Any letters or bags that are not finished with high standards, may be returned to me so that I can make additions and resubmit for credit.

I give my permission for this photo to appear on brochures, official paperwork and Operation Goody Bag and Tomorrow's First Responder website  pages. I understand that no names will be posted near these photos.
